Candidate Questionnaire for Interview: Candidate for Ordination

This guide will help you to prepare for your credentialing interview and will also help the interviewing committee to prepare. You are expected to complete this questionnaire in legible form and submit it to your conference minister/overseer/bishop at least two weeks before the interview date. Expect to take at least the equivalent of a full day of work to complete this, though it is recommended that you work at it in shorter segments over a longer period of time. Copies of your material will be distributed to each member of the interviewing committee before the meeting. If you have not completed the preparation for the licensing interview (see back side), it will be necessary to do so in preparation for ordination. If you have answered those questions in preparation for licensing, the interviewing committee will have access to those documents and you do not need to repeat information that is contained there. You may wish to reflect, however, on anything you want to add or change in those documents in relation to each area below.
